Evaluation of the quality of nursing records of maternal deaths, January to December 2022–2023, Alto Paraná, Paraguay 2024

Nursing records provide evidence of the interventions performed by staff, guarantee continuity of care and patient safety, and provide information of legal, administrative, and research value. The quality of these documents is crucial for care in maternal and child services, especially in contexts of high maternal mortality such as the department of Alto Paraná, Paraguay. The objective was to evaluate the quality of the nursing records of maternal deaths that occurred between January and December 2022–2023 in the Tenth Health Region, Alto Paraná, Paraguay. The research features a quantitative approach with an observational, descriptive, cross-sectional, and retrospective design. The sample consisted of 26 nursing records contained in the clinical files of maternal deaths in the Tenth Health Region, selected through non-probability convenience sampling. The results showed that 100% of the records were prepared by licensed nurses. In the structure dimension, a high level of compliance was evidenced in first and last name (100%), professional signature (96%), and date and time (92%); meanwhile, the recording of medical diagnosis (31%) and norms and protocols (46%) presented the lowest percentages. In the content dimension, legibility reached 96% and the recording of discharge, transfer, or death reached 92%, although the recording of assessment data of the Nursing Care Process (NCP) only reached 27%. In conclusion, the evaluation of the nursing records of maternal deaths showed good quality in the structure dimension (score of 10.5; 75%) and fair quality in the content dimension (score of 9.8; 52%).
